This policy explains what we collect through adirondackaudiology.com, why we collect it, and what we do with it. It covers the website only.
If you are already our patient: the health information we create and hold in the course of your care is governed by HIPAA, not by this policy. That is set out separately in our HIPAA Notice of Privacy Practices (PDF). Where the two overlap, the HIPAA notice governs.
Who we are
Adirondack Audiology Associates operates five hearing care clinics in Plattsburgh, Potsdam and Saranac Lake, New York, and Shelburne and Milton, Vermont. We are the controller of the information described here.
Information you give us
You do not have to give us anything to read this website. You give us information when you choose to contact us.
Free hearing assessment form
Our online hearing assessment asks for your name, email address, telephone number and preferred office, together with your answers about your hearing — the situations you find difficult, whether you experience tinnitus, whether you have had a hearing evaluation before, whether you currently wear hearing aids and of what make, how soon you would like to be seen, whether you have NYSHIP / Empire Plan coverage, and anything else you choose to add in the notes field.
Contact form
Our contact form asks for your name, telephone number, email address, postal address, the reason you are getting in touch, your nearest office and any comments you add.
Web chat
If you use the chat window on the site, we receive the messages you send and any contact details you provide in them.
Some of what these forms ask about — your hearing, tinnitus, insurance — is health information. Please do not send us anything more sensitive than the form asks for. Email and web forms are convenient but they are not secure channels. If you need to discuss something private, call your clinic instead.
Information collected automatically
- Analytics. We use Google Analytics to understand how the site is used — which pages people read, how they arrived, roughly where in the world they are, and what kind of device and browser they use. This is reported to us in aggregate. We do not use it to identify you by name.
- Server logs. Our hosting provider records standard technical information, including IP addresses, for security and troubleshooting.
- Cookies. The site sets cookies that make WordPress work, cookies used by Google Analytics, and a cookie used by the chat window. You can block or delete cookies in your browser settings; parts of the site may then not work as intended.
How we use it
- To answer your inquiry and arrange an appointment at the office you asked for
- To prepare for that appointment, so your time with the audiologist is spent well
- To provide and bill for care, once you become a patient
- To keep the website working, secure and useful
- To meet our legal, professional and record-keeping obligations
We do not use the information you send through this website to build advertising profiles.
Who we share it with
We do not sell your information, and we do not rent or trade it.
We share it only with the service providers who make the practice run, and only so far as they need it to do their job for us:

We may also disclose information where the law requires it, where it is needed to establish or defend a legal claim, or to protect someone’s safety.
Text message consent is never shared. If you give us your mobile number and agree to receive text messages, neither that number nor your consent is shared with, sold to, or rented to any third party for their own marketing. It is shared only with the provider that delivers the messages on our behalf. See our SMS Terms of Service.
How long we keep it
Inquiries that do not become appointments are kept only as long as we need them to respond and to keep a record of the request. Patient records are kept for the period required by New York and Vermont law and by our professional obligations, which is longer. Analytics data is retained on the standard Google Analytics schedule.
How we protect it
The site runs over an encrypted connection (HTTPS), access to form submissions is limited to staff who need it, and our systems are kept up to date. No website can promise perfect security, and we will not pretend otherwise — which is why we ask you not to send sensitive details through web forms or email.
Your choices
- Ask us what we hold. Contact us and we will tell you, and correct anything that is wrong.
- Ask us to delete it. We will, unless we are required to keep it — patient records in particular.
- Stop the texts. Reply STOP to any message. See the SMS Terms of Service.
- Stop the emails. Use the unsubscribe link, or just tell us.
- Turn off analytics. Use your browser’s cookie controls, or Google’s opt-out browser add-on.
Patients have additional rights over their health records under HIPAA. Those are described in the HIPAA Notice of Privacy Practices.
Children
This website is not directed at children and we do not knowingly collect information from children through it. We do provide pediatric audiology; those appointments are arranged by a parent or guardian.
Other websites
Where we link out — to a hearing aid manufacturer, to our financing partner, to Google Maps — that site has its own privacy policy and we are not responsible for it.
Changes
If we change this policy we will update the effective date at the top. Material changes will be flagged on this page.
